What you need to know
A guest can be referred to The Bird House by anyone including their primary care physician, family, or facility staff. Our house manager will contact or visit the guest and their family wherever they are to explain the services of the home and gather any other information about the guest’s needs.
If all the rooms in the house are full, we have a waiting list. When a room becomes available, we will contact the family to discuss details and next steps.
Who is Eligible?
End-of-life care
- Life expectancy of six months or less
- Guest must already be enrolled with one of our partnered hospice agencies
- COVID negative at time of admission
- Patient weight of less than 300 pounds to ensure safe care is provided
- Guest has a “Do Not Resuscitate” status and a Durable Power of Attorney for health care decision making
- There is a hospice plan for continued care in case of discharge
- The guest, their family, or their friends agree on the purpose of admission, hospice care plan, and projected length of stay
